We cannot predict the amount of time that a person will have to wait to be placed in a clinic, but you will be placed as soon as possible.

Due to the high demand for family doctors and nurse practitioners, you must provide at least 24 hours cancellation notice if you cannot make it to your first visit with your new care provider.

While we know that unforeseen circumstances will arise, If you do not show up to your scheduled first visit and do not provide 24 hours notice, your name will be resubmitted to the end of the wait-list.

Please note: this is a waitlist only and does not guarantee you will be rostered to a Physician or Nurse Practitioner. This system is intended to direct registrations to the Physicians/Nurse Practitioners and they accept patients into their practices accordingly. Please be assured that Our Health Team and its employees follow a strict privacy policy to maintain your confidentiality. Definitions
“Primary Care Provider” or “Provider” means a Practitioner, administrative team member, clinic or business which is licensed to practice medicine in the Province of Ontario and has a primary practice in Muskoka.
“Identifying Information” means personally identifiable information that identifies an individual or for which it is reasonably foreseeable in the circumstances that it could be utilized, either alone or with other information, to identify an individual. This includes information such as your name, address, e-mail address and date of birth.
“Non-Personal Information” is information of an anonymous nature, such as an Internet Protocol Address (IP Address), the domain used to access the site, and the type and version of browser or operating system being used. Aggregate information, such as demographic statistics of our users (e.g. average age or geographical allocation of our users), number of visitors, what pages users access or visit, average time spent on the Site, and information volunteered by the user, such as survey information, is also considered Non-Personal Information.
“Personal Information” in the context of this Policy, means Identifying Information.
“User” means members of the public who provide identifying information on SouthMuskokaWaitList.com

By using the Services, you consent to our collection from, verification with and communication to the Provider. Personal Information will not be used without your consent for any purpose other than those mentioned in this Policy.

SouthMuskokaWaitList.com uses commercially reasonable efforts to ensure that your Personal Information is stored and maintained in a secure environment. However, please note that this is not a guarantee that such information may not be accessed, disclosed, altered, or destroyed by breach of any of our physical, technical, or managerial safeguards.
Retention
SouthMuskokaWaitList.com will keep your Personal Information for as long as necessary in connection with the purposes identified in this Policy or as required by law, You must notify SouthMuskokaWaitList.com at [email protected] if you no longer want your information to be retained by SouthMuskokaWaitList.com.
You acknowledge and agree that if you request that your Personal Information be removed from our databases, it may not be possible to completely delete all Personal Information due to technological and legal constraints.
